Facelift + Laser Resurfacing: The 3-Step Blend Patients Don’t Hear About

Aging is rude enough to work on several layers at once. Even when you want Father Time to throw you a bone and give you a few more years without wrinkles, he never seems to listen.

Changes beneath your skin gradually alter the shape of your face, while your complexion follows its own aging process. A facelift may restore your jawline and neck beautifully, yet your results can still feel unfinished because surgery wasn’t designed to correct everything visible at the surface.

When Dr. David Passaretti plans facial rejuvenation, he evaluates how aging has affected your deeper support, facial volume, and skin quality. If you’re showing changes across several tissue layers, he may recommend combining a facelift with selective fat transfer and laser skin resurfacing. Each procedure contributes something different, allowing your face to look refreshed as a whole rather than treated in separate pieces.

Step One: Put the Structure Back

A facelift is designed to correct movement that has occurred beneath your skin.

As your facial tissues descend, the lower face begins to look heavier and the once-clean transition between your jaw and neck becomes less defined. Facelift surgery addresses these changes by repositioning the deeper tissues responsible for them rather than asking your skin to hold everything in place.

Depending on your anatomy and the degree of aging, your surgeon may work with the superficial musculoaponeurotic system, commonly called the SMAS, or use a deep plane facelift technique to release and reposition your facial tissues as a cohesive unit. A neck lift may also be incorporated when excess skin, fat, or muscle laxity extends below your jawline.

Once the underlying structure has been restored, excess skin can be removed with less tension. Your skin then settles over a stronger foundation, helping your facelift results look natural rather than pulled.

Facelift surgery is exceptionally effective at restoring shape, but it can’t sand away sun damage or persuade fine wrinkles to pack their bags. That's where steps two and three come in.

Step Two: Restore Volume Where It’s Faded

Lifting and filling play different roles in facial rejuvenation.

A facelift returns descended tissues to a better position, although it doesn’t automatically replace the facial fat you’ve lost with age. If your cheeks, temples, or transitions around the eyes have become hollow, lifting alone may leave your face looking more defined without fully restoring its former softness.

Fat transfer allows your surgeon to replace volume selectively using a small amount of fat from your own body. Once the fat has been prepared, it can be placed with precision in areas that need additional support.

The goal isn’t to make your entire face look fuller. A restrained fat transfer can soften a hollow transition or improve the way your skin drapes over the newly lifted tissues while preserving the proportions that make your face recognizable.

You may only need very little added volume, or you may have experienced enough fat loss that it becomes an important part of your treatment plan. Your anatomy decides which approach makes sense. Trends don’t get a vote.

Step Three: Treat the Skin Surgery Leaves Behind

Laser skin resurfacing works at the surface, where a facelift stops.

During laser resurfacing, controlled energy removes or heats selected layers of damaged skin. Deeper ablative treatments, including certain carbon dioxide laser treatments, create a more substantial renewal response, while lighter laser treatments may provide more modest improvement with a shorter recovery time.

As the treated areas heal, your body produces new skin and stimulates collagen production. Over time, this healing process can soften fine lines, improve skin texture, reduce visible sun damage, and make certain acne scars less noticeable. Your skin appears smoother and better matched to the cleaner contours created by surgery.

Skin quality has a major influence on how facelift results appear in person. Even with a sharper jawline and a more defined neck, heavily sun-damaged skin can continue to reflect signs of aging. Once the texture and tone improve, light moves across your face more evenly and the result tends to look more cohesive.

One Recovery, Several Healing Timelines

Combining procedures may allow you to complete several stages of facial rejuvenation during one recovery period, but each treated layer will heal at its own pace.

Swelling and bruising from facelift surgery gradually improve over several weeks, while resurfaced skin may initially appear red, swollen, or intensely pink. Deeper laser treatments can require a longer period of visible healing, and some redness may remain after your skin has otherwise recovered. Fat transfer also creates temporary fullness before the surviving fat settles into its final position.

You’ll need attentive aftercare, consistent sun protection, and enough time before photographs, travel, or major events. Infection, delayed healing, pigment changes, and scarring are possible with any cosmetic procedure, making appropriate patient selection and close follow-up especially important.

Take Care of Your Face

A facelift restores your facial architecture. Fat transfer replaces selected volume, and laser skin resurfacing refines the skin covering it all.

For good candidates, this three-step approach can produce a more complete result because it follows the way facial aging actually develops. Your deeper tissues regain support, your contours recover their natural softness, and your skin no longer looks as though it missed the meeting.
